Responsibilities
Clinical main tasks, duties & responsibilities
- Demonstrates clinical competence in the area of nephrology ensuring provision of high-quality safe patient centred care, by setting and monitoring standards of care by example and direction.
- Assess, appraises and critically applies evidence base for clinical practice. Taking an active role in assisting with the implementation of research-based practice and setting and evaluation of unit policies and standards.
- Responsible for the assessment and care of patient’s on a day to day basis
- Is aware of, observes and adheres to local Renal Service policies, hospital, organisational and national policies.
- Is familiar with national guidance and standards such as Quality Improvement Scotland for Adult Renal Services, Scottish Intercollegiate Guidelines Network and Clinical Resource and Audit Group.
- Participates in nursing / clinical audit and identify where clinical practice / standards are not met and assist to implement subsequent improvements in patient care and service provision.
- Ensures all programmes of care are individualised and reviewed on an ongoing basis to maintain high standards and to highlight potential problems ensuring they are acted on promptly.
- Empower patients to take responsibility for their health; well-being and future lifestyle. Ensure patients are given the relevant information and education to participate in the decisions about their care.
- Works within the NMC code of practice and professional guidelines.

Band 2 NHS HEALTH CARE SUPPORT WORKER
To assist in the delivery of care to satisfy patient individual needs. To maintain patients privacy, dignity and comfort whilst promoting independence and encouraging a healthy lifestyle. Scope and range:
Duties and Responsibilities:
- Assisting nursing staff in caring for an individuals hygiene and physical comfort. – Assist in creating a supportive and caring environment.
- Carrying out toileting, including emptying urinary catheter bags.
- Dispose of clinical waste in line with health and safety instructions Undertake clinical activities as directed and under the supervision of the Registered Nurse.
- Measure and record routine observations of temperature, pulse, blood pressure, Weight and, urinalysis..
- Apply simple dressings to speciality definition.
- Assist with patient mobility using correct equipment and techniques.
- Handle and label specimens safely and correctly
- Obtain blood sugar levels using B.M.stix.
- Maintain, monitor and control stock and equipment within the department, reporting any deficit to senior nurse on duty.
- Prepare specific equipment required within the department prior to clinical procedures.
- Assist in protecting patients, visitors and staff from abuse and assist in managing abusive and aggressive behaviour.
- Be responsible for clinical incident reporting according to the Trust’s health and safety standards.
Escort patients to other wards and departments with out direct supervision. Works to well established routine / procedures, may participate in discussions on proposed changes.
Requirements

Full Driving License and own car (preferably.)

Band 3 NHS MENTAL HEALTH CARE SUPPORT WORKER
Responsibilities
To work as part of a care team to deliver evidence-based interventions to service users with mental health problems. To develop and practice, working alongside registered clinicians, a set of therapeutic care skills, making a significant contribution to service users’ psychological, emotional, and social wellbeing. To ensure service users and their relatives receive care that prioritises safety, effectiveness, partnership, and hope. To promote and champion core care values by supporting junior, temporary and newly appointed Healthcare Support Workers.
- To deliver high quality care and recovery interventions for service users within designated clinical areas, in collaboration with, and under the indirect supervision of, registered care staff. These may include:
- Supporting individuals with fluids and nutrition, physical wellbeing, and personal care.
- Assisting service users with financial or accommodation issues.
- Escorting or accompanying service users for planned periods of leave on hospital grounds and in the community facilitating social inclusion.
- Encouraging service users to talk about their experiences in one-to-one or group discussions.
- To help maintain a clean, well-organised, safe and therapeutic environment that meets the needs of service users, carers and colleagues, and to support registered care staff to perform standardised environmental assessments to monitor and improve the care setting.
- To deliver psychosocially informed one to one and group interventions aiming to positively impact service users’ cognitive and emotional wellbeing.
- To work as part of a multi-disciplinary team. To assume lead or link roles on core practice initiatives and priorities (e.g. Essence of Care, Safe wards and carer engagement) under the supervision of registered staff. To integrate areas of personal interest with clinical practice to enhance the diversity of the team’s provision of care.
- 5 To practise planned care, and to directly participate in the development of recovery-focused interventions that address service users’ and carers’ needs and preferences.
- To participate in the on-going care planning, CPA and risk assessment processes in collaboration with service users, their carers, and other members of the care team and to contribute to all aspects of clinical record keeping.
- To maintain a basic working understanding of both the Mental Health Act (1983) and Mental Capacity Act (2005), and any other relevant legislation, as required by mandatory and statutory training (MaST).
